Veteran Directed Care (VDC) (Case Management) SpecialistAIM seeks a Veteran Directed Care (VDC) (Case Management) Specialist to join our mission-driven organization. This position assesses veterans' needs and supports the veteran as they develop their self-directed budget and manage their care needs.Essential Functions:Assist AIM's VDC participants and their authorized representative in self-directing home and care services, while ensuring they stay within their approved budget.Assist veterans with hiring care attendants, monthly and annual reporting, corporate compliance, and general administrative support.Prepare and maintain data in compliance with policies and procedures.Review veterans' budgets monthly to ensure they are within their budget. Address any questions they may have in regard to their budget.Ensure the VDC Manager has the appropriate veteran and attendant information to allow for billing as described within the VDC Billing and Invoices Procedures Guide.Maintain current copies of the veteran's case mix budget, Spending Plan, Emergency Backup Plan, and Authorized Representative designation. Retain these documents for a period of seven years from the date of care, services, or supplies were furnished or billed, whichever is later.Complete and submit quarterly reconciliation to the VDC Manager, detailing actual expenses vs. budgeted expenses.Document all interactions with the veteran and authorized representative, which must include, at minimum, a monthly phone call and a quarterly in-home visit.Assist veterans with the hiring and paperwork associated with their self-hire staff.Perform ongoing recruitment by developing relationships to market AIM Independent Living Center as a VDC service provider.Experience and Qualifications:Associate degree in Human Services or Social Work with 4 years of applicable experience, orAny combination of education and experience that provides the knowledge, skills, and ability necessary to perform the essential functions of this position to the satisfaction of AIM's Chief Executive Officer.Physical and Environmental Requirements:Work is typically performed indoors and requires minimal lifting of up to 10 pounds, and the ability to sit and work at a computer for prolonged periods of time.Extensive reading and researching relevant topics.Travel throughout several counties, including Livingston, Yates, Monroe, Wayne, and Ontario.
