CalOptima Health is seeking a highly motivated an experienced Social Worker (LTSS) (CalAIM) to join our team. The Social Worker Long Term Services Supports (LTSS) CalAIM will be responsible for providing care coordination for members who are enrolled in or requesting CalAIM support services. The incumbent will collaborate with the appropriate LTSS representative and other health care professionals to coordinate referrals and benefits for CalAIM. The incumbent will assist in facilitating collaboration between CalAIM and the Orange County Social Services Agency (SSA) In-Home Support Services (IHSS).

Salary Grade: 309 - $68,015 - $108,824 ($32.70 - $52.3192)
Work Arrangement: Partial Telework**This position is eligible for telework in California.**
Duties & Responsibilities:
75% - Care Management
Participates in a mission-driven culture of high-quality performance, with a member focus on customer service, consistency, dignity and accountability.
Provides education to members, families and providers about CalAIM.
Conducts CalAIM personal care services assessments with members.
Reviews CalAIM assessments completed by other entities.
Coordinates with SSA-IHSS to ensure member's needs are met which includes, but is not limited to, the following: refers members to IHSS, requests IHSS complete another assessment when a change of condition occurs, coordinates with IHSS on status of pending IHSS requests, assists members in resolving barriers to obtaining IHSS, and coordinates with the IHSS Public Authority if the member needs help finding an IHSS provider.
Coordinates with recuperative care and other community supports.
Coordinates with waiver organizations.
Reviews housing support documents such as tenant screenings, housing assessments and individualized housing support plans to ensure housing support services are reasonable and necessary.
Follows up with housing providers to ensure all required documents are submitted to CalOptima Health.
Documents all contacts and case information in the appropriate case management systems using standard charting formats.
Develops and maintains a network of current community resources and services where CalOptima Health members can be referred for assistance.
20% - Support Services
Assists the team in carrying out department responsibilities and collaborates with others to support short- and long-term goals/priorities for the department.
Retrieves and monitors incoming inquiries from various sources (i.e., Jiva case management system, CalOptima Health Connect Portal, email, etc.)
Participates in training of internal staff and works collaboratively with other CalOptima Health departments to meet member's needs.
Acts as a liaison between the LTSS department, other CalOptima Health departments and providers to assist with issues, concerns and inquiries.
Ensures confidentiality of all protected health information (PHI).
5% - Other
Completes other projects and duties as assigned.
Minimum Qualifications:
Master's degree in behavioral science (i.e., social work, sociology or psychology) PLUS 2 years of experience working with the needs of SPDs in a customer/member service capacity required; an equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above may also be qualifying.
Preferred Qualifications:
HMO, Medi-Cal, Medicare and health services experience.
Experience performing psychosocial assessments.
Experience working with members in a multi-disciplinary care team setting.
Bilingual in English and in one of CalOptima Health's defined threshold languages (Arabic, Farsi, Chinese, Korean, Spanish, Vietnamese).
