DESCRIPTION

The City of Long Beach Health and Human Services Department, Community Health Bureau has an opening for a Public Health Professional III. This position will be split between the Black Infant Health (BIH) program (50%) and the Healthy Aging Center (Senior Links Program) (50%).

Black Infant Health Program
The Public Health Professional III (PHPIII) reports to the Maternal Child Health Director and is under the general supervision of the Black Infant Health (BIH) Coordinator. The PHP III is primarily responsible for conducting participant enrollment activities and assessing client mental health and psychosocial issues. The PHPIII works collaboratively with members of the BIH team to identify behavioral tendencies that impact participant’s abilities to function and makes recommendations for cognitive behavioral approaches that integrate client strengths. The PHP continuously develops relationships with community resources and facilitates linkages to BIH participants.

Healthy Aging Center (Senior Links Program)
The Public Health Professional III (PHPIII) reports to the Healthy Aging Center Public Health Nurse III. This Public Health Professional III (PHPIII) is responsible for the provision of social work services that include but is not limited to Targeted Case Management (TCM) services, participant/caregiver support, contributing to assessments and plan of service, assisting participants/caregivers to assess and utilize community resources. The goal is to provide seniors with the support they need to live independently and safely in their homes with the focus on the preservation and/or enhancement of quality of life.
