DESCRIPTION

The City of Long Beach Health and Human Services Department, Community Health Bureau has an opening for a Public Health Professional III. This position will be split between the Black Infant Health (BIH) program (50%) and the Healthy Aging Center (Senior Links Program) (50%).

Black Infant Health Program
The Public Health Professional III (PHPIII) reports to the Maternal Child Health Director and is under the general supervision of the Black Infant Health (BIH) Coordinator. The PHP III is primarily responsible for conducting participant enrollment activities and assessing client mental health and psychosocial issues. The PHPIII works collaboratively with members of the BIH team to identify behavioral tendencies that impact participant’s abilities to function and makes recommendations for cognitive behavioral approaches that integrate client strengths. The PHP continuously develops relationships with community resources and facilitates linkages to BIH participants.

Healthy Aging Center (Senior Links Program)
The Public Health Professional III (PHPIII) reports to the Healthy Aging Center Public Health Nurse III. This Public Health Professional III (PHPIII) is responsible for the provision of social work services that include but is not limited to Targeted Case Management (TCM) services, participant/caregiver support, contributing to assessments and plan of service, assisting participants/caregivers to assess and utilize community resources. The goal is to provide seniors with the support they need to live independently and safely in their homes with the focus on the preservation and/or enhancement of quality of life.

EXAMPLES OF DUTIES

Black Infant Health Program:

• Conducts participant enrollment activities, which include orientation, informed consent, and the initial assessment.
• Completes assessment form during Life Planning meetings with clients to determine appropriate referrals to Medi-Cal services.
• Refers participants to Family Health Assessment Services for on-going case management.
• Participates in conference calls with BIH team and other agencies to better serve Medi-Cal and Medi-Cal eligible participants to improve access to Medi-Cal services for high-risk, pregnant and postpartum clients.
• Develops and maintain relationships with local mental health professionals for participant referrals.
• Consults with Medi-Cal mental health providers regarding clients’ mental health needs.
• Participates in group sessions that focus on mental health issues by being available to answer participant questions and provide support to group facilitators.
• Provides mental health education and services to clients.
• Conducts trainings on maternal and infant mental health.
• Helps develop protocols that address clinical issues and mental health services.
• Attends interagency meetings to discuss ways to reduce barriers and increase participation in Medi-Cal funded services.
• Attends expert trainings and professional education in-services relevant to the role of the mental health professional and to the administration of BIH Programs to facilitate access to Medi-Cal services.
• Completes all required trainings including Technical Assistance calls with the State BIH program, attends required meetings, and adheres to the BIH Policies and Procedures.
• Attends trainings related to health disparities, Federal Financial Participation and Medi-Cal services.
• Develops a Professional Development Plan in conjunction with the BIH Coordinator to reflect basic skills of the Health Resources and Services Administration Maternal Child Health Bureau Leadership Competencies.
• Participates in the quarterly Federal Financial Participation time study to account and support all work activities.

Healthy Aging Center (Senior Links Program):

• Provide Targeted Case Management (TCM) and comprehensive social work case management services.
• Conduct client intake and comprehensive psychosocial assessments of older adults to identify needs related to health, housing, finances, and social support.
• Develop and implement individualized care plans in partnership with clients, coordinating services with healthcare providers, family members, and community agencies.
• Advocate for clients' rights and needs, ensuring access, referral, and linkages to appropriate services and benefits like Medicare and Medicaid.
• Conduct home visits to monitor client well-being and assess their living environment.
• Participates in case conferences and collaborates with multidisciplinary teams including medical professionals, nurses, and other social service providers.
• Support and assist clients in applying/maintaining services such as Medi-Cal, Medicare, Cal Fresh, In-Home Supportive Services, SSI/SSDI, etc.
• Maintain working knowledge of and liaison with community agencies and resources.
• Provide supportive counseling to clients and their families navigating challenges of aging, including grief, depression, dementia, health conditions, and life transitions.
• Provide crisis intervention, counseling and emotional support to clients and their families navigating challenges of aging, including grief, depression, and dementia.
• Educate clients and families on aging-related issues, including end-of-life planning, care transitions, and decision-making.
• Provide clinical supervision of MSW graduate student interns, as appropriate.
• Attend and participate in staff, committee, department, and other meetings.
• Other duties as assigned.

REQUIREMENTS TO FILE

EDUCATION:

• Graduation from an accredited college or university with a Master of Social Work (*proof required).
  • Candidates may substitute experience offering specific and substantial preparation for the duties of the position for the required education on a year-for-year basis.

EXPERIENCE:

• A minimum of one (1) year of full-time professional experience in public health or human services related to a specialized program area.

ADDITIONAL REQUIREMENTS:

• A valid motor vehicle operator's license to conduct home visits as needed for the position.

DESIRABLE QUALIFICATIONS
Black Infant Health Program:

• Understands and respects the values and beliefs of Black/African American women and the Black/African American community.
• Fully embraces and supports the BIH Governing Concepts of cultural competence, client-centered, strength-based, and cognitive skill-building approaches.

Healthy Aging Center (Senior Links Program):

• Licensed Clinical Social Worker (LCSW)
• Professional or academic experience focused on Gerontology.
• Bilingual skills in English, Spanish, Khmer, or Tagalog.

KNOWLEDGE, SKILLS, AND ABILITIES
Skills/Abilities:

• Cultural competence and ability to operate in a culturally-affirming manner.
• Clinical experience in individual and group psychotherapies dealing with individuals in multiple health and social systems.
• The ability to identify behavioral tendencies that impact functioning, and recommend cognitive behavioral approaches that integrate client strengths into improving functioning.
• The ability to conduct crisis intervention as needed or as resources allow.
• Sound communication and interpersonal skills.
• Critical thinking and problem-solving skills.
• Basic administrative skills (i.e. appointment scheduling, documentation, report writing, computer skills).
• The ability to work collaboratively within a multidisciplinary team.

Black Infant Health Program:
Knowledge of the following:

• Women’s health, including prenatal and postpartum mental health and psychosocial issues.
• Life course perspective.
• Impact of psychosocial, cultural and economic factors on the health of women, their infants and their families.
• Psychosocial risk status of BIH participants, including stress, health, social relationships, environment (housing), financial status, transportation that may impact their pregnancies or participation in the BIH Program.
• Prevailing trends and policies in mental health, public health and public welfare.
• Knowledge, understanding and location of community services, including, but not limited to: social services, mental health, substance abuse treatment programs, domestic violence programs, legal systems, housing and other resources for referral.

Healthy Aging Center (Senior Links Program):

• Proven experience working with older adults in a healthcare setting, community-based organization, or similar environment
• Strong understanding of aging issues, including physical, cognitive, and emotional challenges
• Excellent communication and interpersonal skills to build rapport with clients, families, and other professionals
• Ability to conduct assessments, develop care plans, and manage complex cases
• Commitment to ethical practices and client confidentiality
