Pay: $47,824.32

Examples of Work:

• Provide medically related social services for Veterans to attain highest physical, mental and psychosocial well-being through the assessment and care planning process
• Identify and seek ways to support Veterans’ individual needs and preferences, customary routines, concerns and choices
• Make arranges for obtaining adaptive equipment, clothing, and personal items
• Maintain contact with families to report on overall health and planning
• Assist with building relationships between Veterans and the nursing team to promote actions that maintain or enhance a Veteran’s dignity, recognizing individuality
• Identify Veteran’s needs and find options to meet those physical, social, and emotional needs
• Make referrals and obtain services from outside entities.
• Perform duties of Licensed Clinical Social Worker when absent from facility

Qualifications:

Must demonstrate the following which may be obtained through work experience as a social work or human services professional or through completion of a bachelor’s degree in a related program:

• Knowledge of individual, family, and group behavior.
• Knowledge of available community resources.
• Knowledge of medical and treatment records, progress notes, and other related information.

AND

• 2 year of relevant work experience
