THE DEPARTMENT: Ambulatory Care is a system of primary care and specialty care medical clinics conveniently located throughout the County of Ventura. These clinics provide care on an outpatient basis at all levels. Our dedicated team of healthcare professionals offer a range of services tailored to meet the specific needs of our community members experiencing homelessness- from basic medical care and preventative screenings to mental health support, addiction services, social services, shelter, and basic needs. Through partnerships with local agencies, we aim to provide medical treatment and support in navigating the complex challenge of homelessness. We foster a safe and welcoming environment where every patient feels valued, respected, and is treated with dignity and compassion.

THE POSITION: Under immediate supervision, the Community Services Worker assists professional staff in providing transportation, referral, translation and program related services to culturally, economically, physically or mentally impaired or disadvantaged individuals in the community; and performs other duties as required.

THE IDEAL CANDIDATE: The ideal candidate for this is a compassionate, community-oriented individual with experience or education in public service, social sciences, or a related field, and a demonstrated commitment to serving diverse, underserved, and homeless populations. They are skilled in building rapport with individuals from a variety of cultural, social, and economic backgrounds and are adept at providing supportive services such as client transportation, translation, outreach, and referrals to medical, legal, housing, and employment resources. The candidate is knowledgeable about local community structures and resources, understands the unique challenges faced by homeless individuals, and is effective in addressing barriers to care by linking clients to both internal and community-based programs. They are organized, dependable, and capable of documenting services, conducting basic health screenings, and leading group presentations on health, nutrition, and life skills. Bilingual ability. A strong ability to work independently and collaboratively within a care team are highly desirable.

The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary, Fixed-Term), Intermittent and Extra Help vacancies for this position only. There is currently one regular full-time vacancy within Ambulatory Care

Typical Qualifications

EDUCATION, TRAINING, and EXPERIENCE:

Any combination of education and experience which has led to the acquisition of the required knowledge and abilities. The required knowledge and abilities also can be obtained by:

One (1) year of public service experience in providing services to the general public or equivalent experience/education.

Note: Public services are those services provided to the general public by a public non-profit community service organization or a government entity.

Substitution: Fifteen (15) semester units of college course work in social sciences, ethnic studies, communications, or a closely related field may be substituted for six months of experience up to a total of one year.

NOTE: If you are substituting education for experience (up to one year), verification of the college-level coursework is required.

SUPPLEMENTAL QUESTIONNAIRE - Qualifying: All applicants are required to complete and submit the questionnaire for this exam at the time of filing. The supplemental questionnaire may be used throughout the exam process to assist in determining each applicant's qualifications and acceptability for the position. Failure to complete and submit the questionnaire may result in the application being removed from consideration.

APPLICATION EVALUATION - Qualifying: All applications will be reviewed to determine whether the stated requirements are met. Those individuals meeting the stated requirements will be invited to the written examination.

WRITTEN EXAM - 100%: A job-related written exam will be administrated to test applicants’ knowledge of Language Usage, Telephone Etiquette, Community Resources, Interpersonal Relations, and Filing. Applicants must earn a score of seventy percent (70%) or higher to qualify for placement on the eligible list.

Applicants successfully completing the exam process may be placed on an eligible list for a period of one (1) year.

BACKGROUND INVESTIGATION: A thorough pre-employment, post offer background investigation which may include inquiry into past employment, education, and driving record may be required for this position.
