Why you’ll love this position:Hawthorn Children's Psychiatric Hospital is seeking a dependable, adaptable,, and team-oriented full-time case manager, who will play a critial role in coordinating care for youth requiring admission for Residential and Inpatient psychiatric treatment.
This dynamic position requires flexibility and willingness to work where support is most needed across the facility. This case manager is a member of the Social Service Department. They will provide case management for the Intake Department by helping with admissions and being a first point of contact regarding potential clients, gathering essential information to help determine eligibility and appropriate service needs. They will support the multidisciplinary Inpatient Treatment Team by serving as a point of contact for families, guardians, and community mental health agencies among other duties. They will assist with referrals and discharge planning and will help support increased family interactions.
This position reports to the Inpatient Treatment Coordinator and Chief of Clinical Services. The sched ule is generally 8:00 am to 4:30 pm Monday through Friday, with the ability to flex schedule at times. This could be necessary to accommodate family treatment sessions, potentially one evening during the week to meet the needs of patients and families.
The qualified candidate will have a Bachelor’s Degree in Social Work from an accredited university. They will possess k nowledge and of individual, family, and group behavior; current social and economic challenges; and availability of community resources. They will also understand medical and treatment records, progress notes, and related information. They will have computer skills such that they can document in an electronic health record and other documents.
